Verifying Eligibility

Network Information
NCD is a PPO plan that utilizes the MaximumCare PPO networks, including Careington, Connection Dental, and Dentemax. Our plans cover 100%* for preventative services, 80%* for basic services, and 50%* for major services after the 12-month waiting period. Some members may have the waiting period waived with qualifying proof of prior coverage.

Direct Fax Back Service
The easiest way to verify eligibility is to utilize the Direct Faxback Service at 480-550-6440.

MBA Provider Portal
You can visit the claims payer website at mbaadmin.com and click on Provider Eligibility. Enter your Provider Tax ID number and the patient’s Member ID number. This will allow you to see benefit information, coverage details, confirm if the waiting period has been waived, and view maximum remaining amounts.
